Looking for some direction or advice on a problem area? Sign up for a PT Injury Quick screen and let us help direct you to the right health care provider/facility. This is a screening process for an acute or chronic injury triage. Dr. Greene will take a subjective and then triage to the appropriate service for you or your athletes need. This service is offered virtually as well! Please specify in the "comments" if you would like this Screen to be virtual. ***There is a $8 additional gym fee and a premium rate for all Saturday Appointments! This appointment type is not reimbursable by insurance and will therefore not be itemized into a Superbill. Please email admin@perfect10pt.org for specific pricing questions for this service. ATTN: This service is not covered my government issued insurance (ie. Medicaid, Medicare Part A and B and Tricare) and therefore is not reimbursable and will not be itemized into a Super Bill.
